Please only proceed with scheduling if you are able to comply with the following policies. This ensures the best experience for you and all our patients.

Email Is Our Primary Method of Communication

We send important appointment confirmations, reminders, and required forms via email. It is your responsibility to ensure you are receiving emails from us and that our messages are not going to your junk or spam folder. Please check your inbox settings and whitelist our email address.

Electronic Forms Are Mandatory

Our practice uses electronic forms to streamline your care:

  1. Insurance Verification Form – You will receive this form immediately after booking. It must be completed within 48 hours of booking your appointment.

  2. New Patient Medical History Intake Form – This will be emailed to you once your insurance is verified. It must be completed at least 24 hours before your appointment so we can best prepare for your visit.

Completion of both forms is required to proceed with your visit. Failure to submit either form by the required deadline will result in automatic cancellation of your appointment.

Card on File Required

A debit or credit card is required for the purpose of holding your reservation. Your card will not be charged during this process. Missed appointments without prior notice will incur a $35.00 fee. Please notify us by phone if you are unable to attend your appointment.

Acupuncture Services

Acupuncture is a medical specialty which profoundly reduces inflammation, induces analgesia (pain relief), decreases stress hormones, and stimulates healing through the insertion of very thin needles at strategic anatomical locations on the body. A key component of traditional Chinese medicine, Acupuncture is most commonly used to treat pain syndromes.

Cameron is a licensed, board-certified Acupuncturist and East-Asian Medicine Practitioner. His advanced didactic and clinical training includes orthopedics and neuromusculoskeletal pain management. He is a diplomate of Acupuncture with the National Certification Commission of Acupuncture & Oriental Medicine (NCCAOM). His post-baccalaureate clinical training includes externships in Pain Management at UCLA Health: Venice Family Clinic.
