Booking Policy:
A debit / credit card is required during the scheduling of your initial visit for the purpose of holding your reservation. Your card will not be charged during this process. Missed appointments without notice will be charged a $35.00 fee. Please notify us by phone or text if you are unable to keep your appointment.
Upon scheduling your initial appointment, you will immediately receive an email containing an electronic insurance intake form. Completion of this form is required for us to proceed with your appointment as we need to verify your insurance policy details for acupuncture benefits and coverage. If this email does not show up in your inbox, please check your spam folder.
