IMPORTANT BOOKING POLICY
Please note that our clinic is near capacity and currently only accepting new patients with a referral from a Primary Care Provider (PCP). An active referral is required before scheduling. Referrals can be faxed to our office at (509) 931-0484. Only proceed with booking if you are able to comply with the following clinic policies. Thank you for helping us maintain an efficient practice for all patients.
1. Email Communications
Consent to receive email communications is required for online booking and to receive care at our office. Our office corresponds with patients via email regarding scheduling, appointment confirmations, reminders, and required forms. It is the patient’s responsibility to ensure that emails from our office are successfully received and not filtered into junk or spam folders. Please check your email settings and whitelist our email address to ensure proper delivery of communications.
2. Electronic Intake Forms
Our practice uses electronic forms to streamline your care. Completion of both forms is required to proceed with your visit. Incomplete submission of either form by the required deadline will result in automatic appointment cancellation.
New Patient Insurance Verification Form – You will receive this form immediately after booking. It must be completed within 48 hours of booking your appointment.
New Patient Medical History Intake Form – This will be emailed to you once your insurance is verified. It must be completed at least 48 hours before your appointment so we can best prepare for your visit.
3. Card on File
A debit or credit card is required for the purpose of holding your reservation. Your card will not be charged during this process. Appointments missed or canceled with less than 24 hours’ notice are subject to a $50.00 fee. Please notify us by phone if you are unable to attend your appointment.
